- Teams: Varsity, JV and Developmental (depending on number of athletes)
Sports Physical Form (this is the only form we are able to accept for participation)
- must be submitted prior to the first practice
- send form to our Athletic Director, Natalie Johnson (make sure student's name is on it)
- note: if you are registering for a second or third sport season, you do not have to submit another physical unless yours is expired
Required equipment:
- Basketball shoes/athletic shoes
Practice Pickup: Gym doors (note: no entry to the school is permitted during pickup)
