Please complete this form and submit for approval a minimum of two weeks prior to departure.
 
Elementary Forms/Policies (Grades K-4th)
 
Elementary Beginning of the Year Form (completed through Express Check-in)
A link to this required form will be found in DCSD Express Check. This is a required electronic form for all elementary students. This includes our Acceptable Use Policy, Dress Code Agreement and Volunteer Agreement. Students without this form on file will be unable to use any school devices or technology.
 
Middle School Forms/Policies (5th-8th Grades)
 
Middle School Beginning of the Year Form (completed through Express Check-in)
A link to this required form will be found in DCSD Express Check. This is a required electronic form for all middle school students. This includes our Locker Agreement, Acceptable Use Policy, Dress Code Agreement, Volunteer Agreement, and Cell Phone Policy.. Students without this form on file will be unable to use any school devices or technology and will not be assigned a locker until the form is completed.
